Forms Overview
Forms provide a way of sending permissions slips, questionnaires, data collection sheets, etc. Forms can include both information and questions that can be completed.
Forms sent using IRIS Reach allow you to:
- View and analyse responses
- Send reminders to a person A parent, legal guardian, carer, or someone responsible for a student. who needs to complete a form
IRIS Reach Forms are typically used for:
- Request a person to book a place for a club, register interest for an event or information, or book activities before an event starts
- Send surveys, request votes, gather interest for an event or idea, or to get feedback about your organisation A school, multi-academy trust (MAT), single academy trust (SAT), club, nursery or organisation that uses the software.
- Collect content, emergency contact, allergy, or specific details for a activity, event, or trip
- Keep contact and personal details up to date
When a form has been created, it can be sent to a person about a student. For more information, go to Create and Edit Forms or Send Forms.
When the person has submitted their responses to the form, you can view the responses. Alternatively, if a person is unable to complete the form online, you can manually add the response for them. For more information, go to Manage Form Responses.
