Manage Form Responses

When a person A parent, legal guardian, carer, or someone responsible for a student. responds to a form sent from IRIS Reach, you can view their response. Viewing form responses allows you to review the data collected, for example, consent for students to attend an event or go on a trip. Responses can be viewed for active and archived forms.

  1. Select Parent Reach Module for IRIS Reach used to send MyEd app messages, texts, or emails to parents and carers for individual or all students. > Form Manager.
  2. The Parent Reach - Form Manager page is displayed. In Manage Active Forms or Manage Archived Forms, select View Responses next to the Name of the required form.
  3. The Parent Reach - Form Manager - View Responses page is displayed.

    Form responses are displayed depending on the status. The following statuses are available: 

  • Pending Responses — a form has been sent but a response has not been received
  • Completed Responses — a response has been received for the form
  • Confirmed Responses — you have confirmed the response has been received and viewed

The total number of responses for the status are displayed in the relevant section. If you need to find a particular responses, enter the name of the student in Search for the required section.

Pending Responses

View a Response

  1. In Pending Responses, select the required student Name.
  2. The Parent Reach - Form Manager - View Response page is displayed. You can view the following information: 

    • Form Recipient — the name of the student (select to view the Student Report)
    • Form Sent — date and time the form was sent
    • Form Status — whether the person who the form was sent to has viewed it
    • Form Viewed — date and time the person viewed the form
    • Completion Method — whether the form was sent online or manually

Add a Response

If a person has provided a manual response, e.g., verbally or manually in writing, or they are unable to complete the form online, you can add the response to the form for a particular student.

  1. In Pending Responses, select Manual Data Entry for the student Name.
  2. A copy of the form is displayed. Enter the required details, then select Submit.

Completed Responses and Confirmed Responses

View a Response

  1. In Completed Responses or Confirmed Responses, select the required student Name.
  2. The Parent Reach - Form Manager - View Response page is displayed. You can view the following information: 

    • Form Recipient — the name of the student (select to view the Student Report)
    • Form Sent — date and time the form was sent
    • Form Status — whether the person who the form was sent to has viewed it
    • Form Viewed — date and time the person viewed the form
    • Form Completed — date the responses were submitted
    • Completion Method — whether the form was sent online or manually
    • Returned Form — displays the details of the submitted response

Confirm Responses

When you have viewed the responses to a completed form, you can mark it as confirmed.

In Completed Responses, select Confirm for the student Name. Alternatively, select Confirm All Completed Responses. The responses is moved to Confirmed Responses.

Remove Response Confirmation

If you have confirmed a form response but need to remove the confirmation, in Confirmed Responses, select Un-Confirm for the student Name.

Print Completed Responses

From Print Completed Responses, select Print All Completed Responses to download a PDF of completed responses that can be printed.

Export Completed Responses

From Export Completed Responses, select Export Completed Responses to download a CSV file that contains all the details of completed responses.

To configure the details that are exported, select Start. The Parent Reach - Form Manager - Export Results Settings page is displayed. Select the responses you want to include, then enter a name for the column heading in the pop-up message. Select Save to confirm your selections.

Send Messages About Forms

If a form has not been completed, you need additional information, or just need to contact the person who responded to the form, a message can be sent from IRIS Reach.

  1. In Pending Responses, Completed Responses, or Confirmed Responses, in the Select column, select the required students.
  2. Select Send Message, Send Letter Automatically creates and downloads a PDF version of the message., or Send Email. For more information, go to Compose Parent Communications.
