Create Forms
Step 1
Send a form when you want to send out questionnaires, surveys, collect data, or record consent. Set up a form using sections that you can customise based on the type of form you want to create.
- Go to Forms, then select Create.

From the Form Editor Toolbox, drag and drop the section types you want to use onto the Form Preview.
If recipients are required to submit responses for the form you are sending, we recommend adding a form section that contains the date responses must be received by.

To enter the information and questions, hover over the relevant section and select (edit).
Sections can be copied or deleted as required — hover over the relevant section and select (copy) or (delete).
- To choose who to send the form to and deadline dates to complete it, select Submit.

Did you know?
Selecting Save as Template when creating a form saves a copy you can edit to create other forms or send at a later date.
