Add a Mandatory Consent Option to Forms

To add the option to collect mandatory consent for parents and carers when creating a form:

1. Select Mandatory Option Button.
2. Enter a name for the Option Button Group.
3. Add Yes as the Option Button Value.
4. Select Mandatory Option Button again.
5. Use the same name in Option Button Group.
6. Add No as the Option Button Value.
