Find Forms and Surveys

To access forms and surveys sent by organisations: 

1. Go to Surveys & Forms.

2. Find the relevant form and select to open it.

   Forms and surveys you have already answered can only be accessed until the expiry date. If the form or survey you want to access is no longer displayed in your account, contact the organisation who can confirm the answers you provided.

3. Respond to the form.

If you are using the IRIS ParentMail app, when a form or survey has been sent, an app notification is displayed.
