An Internet return rejection will be in the form of a box number (usually) and an associated error number. Every rejection notice will contain at least two codes the first of which will be error 3001 which simply means the return has been rejected each subsequent code , the most common of which are listed below, will give an indication of what has caused the rejection.
Authentication Failed- This means the user id, password or UTR is incorrect. Check the user id and password by attempting to log into www.gateway.gov.uk with the information in the Setup screen (note that if the user id or password has been changed since running the original Internet return, the electronic copy of that return will contain the old user id and password). The UTR must be on the HMRC list. Check the client list on the Government Gateway use <Ctrl> F to find the UTR, if it is not on the list a 64-8 must be submitted.

6010 in element type Line does not match the minimum length facet of.... This error occurs where a blank line is being included as part of an address associated with this client. In order to resolve this error check each address, if an empty line is present within the details, click into this line then press the delete key followed by the backspace key on the keyboard
377 in box A12, 426 in box A14, 378 in box A15. In order to resolve these errors please ensure that the clients address is complete, the address must a town, county and post code
492 in box 1 on TCS1 (2007 : 18.3) this occurs where the liability calculated by the revenue is different from the figure that iris has calculated. Contact IRIS support for further information.
2015 Duplicate return. A successful return has already been submitted. If there amendments to the return, the revised return must be sent by post.
2021 IR Mark incorrect. This will be a software error, contact IRIS Support.
Once the relevant changed have been made to the clients details, it will be necessary to reprint the electronic tax return in order to create a new file to be transmitted.
