The company number and full company name are required first, followed
by the Relevant date and the Registered date (which is optional).The applicable
update to either the company or person statement must be ticked.
To activate the form go to Edit

Effective date of change | Log. This saves the changes and
activates a completed PSC09 Form.
To display the form go to Company Forms | highlight PSC09, PSC Statement Changed | Print Form.
To display a form with basic details go to Reports | Forms with Basic Details | Form PSC09 –Update to PSC statements. The form opens in Microsoft Word showing the basic details.
To display a blank form, go to Reports | Blank Forms | Form PSC09 – Update to PSC Statements. This opens the completely blank form in Microsoft Word.
