Form 128(4) - Notice of assignment of name or new name to any class of shares

This form must be delivered to Companies House (no fee is payable) within one month when a company assigns a name or new name to any class of its shares except by amending the memorandum or articles or by a resolution or agreement.

To Activate the Form

To activate the completed form go to Edit | Share Register, select Events tab.  Click New | Share Reclassification. Enter the necessary details, once completed click OK. This will automatically produce a Form 128(4).  Go to Reports | Company forms and select the form 128(4) then click Print.

To Display the Form

To display a form with basic details go to Reports | Forms with Basic Details | Form 128(4) - Notice of assignment of name or new name to any class of shares. The form will open in Word showing the basic details which are the company name, the registered number and the Companies House contact details.

To display a blank form go to Reports | Blank Forms | Form 128(4) - Notice of assignment of name or new name to any class of shares. This will open the completely blank form.
